About Select-A-Form

Select A Form Inc. opened in 1986 in Ronkonkoma, Long Island NY. We started as a short to medium run business forms manufacturer strictly for the trade.

Under the roof of a 6,000 square foot building, our business started with two presses, two snap out collators, a bindery department, prep department and front office staff. Within 1 1/2 years, business boomed when we relocated to Holbrook, NY to a 26,000 square foot building adding 3 additional presses and one more snapout collator.

In 1988, Select A Form decided to go into continuous computer forms, adding two additional presses and a continuous collator. From the days Select A Form Inc opened its doors, leading to today, we are fortunate to service our customers and continue to build a great reputation for short to medium quantities.

In addition to our successes, in 2005 we added a 4 color laser, letterhead forms press to service the advancements of current technology. After 34 years, Select A Form Inc. continues to be a premier leader in the forms and laser printing industry.
